I decided to start with some bloodwork just to see where I was at, and when I saw that my testosterone was
I decided to try the oral TRT+. I was skeptical at first because TRT shuts down your HPT axis, and can potentially mess with your fertility. That's a non-negotiable. But given that I knew where my friend lived (just kidding, I trust him), I begrudgingly enrolled in a trial for a TRT compound that maintains the HPT axis.
Best decision of my life. The dad bod is no more, I have switched up my diet, exercise is satisfying, my sex life is back, and I feel confident in every aspect of life. NO NEEDLES! I just started their oral TRT+ protocol a month ago during their soft launch. My mood has been great, strength has been phenomenal, and I have been more productive than ever before. Who is this protocol for?

How does the protocol work and how do I take it?
You’ll use injectable testosterone (subcutaneous or intramuscular) 1–2× per week, paired with hCG as prescribed. Your Maximus clinician sets your dose and schedule and adjusts based on labs and symptoms.
Will I still maintain fertility on TRT + hCG?
TRT alone can suppress LH/FSH and lower sperm production. Adding hCG mimics LH to help support testicular function and reduce the risk of fertility suppression while on TRT. Responses vary and aren’t guaranteed; we check in regularly and adjust your HCG dose according to your symptoms and overall response.
How quickly will I notice results?
Individual results may vary; Most men feel improvements in the first few weeks. Results continue to refine over the first 1–3 months as dosing is optimized.
How will this impact total and free testosterone?
When total testosterone rises under clinician titration, free testosterone typically increases as well, depending on individual SHBG/albumin levels.
Which side effects are possible and how are they managed?
Possible effects include acne, changes in libido, hair shedding if predisposed, rises in hematocrit/hemoglobin, and estrogen-related symptoms (e.g., breast tenderness). We monitor labs and manage as needed (dose adjustments, timing, or add-on therapies when clinically appropriate).
Which labs are required and how often? Can I bring my own?
Baseline labs typically include Total T, Free T (or SHBG), Estradiol, LH/FSH, Hematocrit, and PSA. Follow-up labs are then required after month 1, month 3, then periodically. You may bring your own labs if they include the necessary markers and were conducted within the last 6 months. You can upload them during the signup process. If you don't have them, we can test them for you with an easy to use at home testosterone blood test kit.
I’m currently on TRT, clomiphene, or enclomiphene. How do I transition?
Let your provider know about your current treatment and don’t stop or change medications before consulting us. Your clinician will design a safe transition plan and may time any pauses around labs if needed.

How do self-injections work?

Subcutaneous vs. intramuscular—what route do you use?
Both routes are effective and available through Maximus, but we typically recommend subcutaneous injections. Subcutaneous injections use smaller needles, allow more frequent dosing, provide steadier levels, and are overwhelmingly preferred by people using the therapy.
