Dual GIP and GLP-1. Once weekly.
Tirzepatide is a single-molecule dual GIP and GLP-1 receptor agonist. Mechanism: activates both GIP and GLP-1 receptors, reducing hunger, slowing gastric emptying, increasing satiety, improving glucose regulation and fat metabolism. GIP may further regulate food intake when added to GLP-1 per nonclinical studies.

Titration to maintenance

- Suicidal behavior and ideation (monitor)
- Diabetic retinopathy complications (in T2D clients with pre-existing retinopathy)
- Thyroid C-cell tumors (rodent data). Boxed warning class effect: contraindicated in clients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

What's the difference between the KwikPen® and the vial?
The KwikPen® holds all four weekly doses for the month in one multi-dose pen. The vial is a single-dose presentation. Both contain the same medication at the same self-pay price via LillyDirect. The KwikPen® means one device per month instead of four separate injections from a vial and syringe.
Zepbound® vs Wegovy®— which loses more weight?
In SURMOUNT-5, a 72-week head-to-head trial sponsored by Eli Lilly, average body weight loss was 20.2% on Zepbound® vs 13.7% on Wegovy®. Waist circumference reduction was 7.2 inches vs 5.1 inches. Wegovy® is the only one of the two with the FDA-approved cardiovascular risk reduction indication. The right choice depends on your goals and clinical history.
What is the OSA indication?
Zepbound® is FDA-approved as the first and only prescription medicine for moderate-to-severe obstructive sleep apnea in adults with obesity. In SURMOUNT-OSA, AHI was reduced by up to 62.8%, with OSA disease resolution in 43.0% to 51.5% of adults studied.
How long do results last?
In a 3-year extension of SURMOUNT-1, adults sustained an average 22.9% body weight loss on the 15 mg dose. As with any medication for chronic conditions, maintenance is part of the protocol.
What about pre-diabetes?
In a SURMOUNT-1 extension in adults with pre-diabetes and obesity or overweight, Zepbound® showed a 94% reduction in the risk of progression to type 2 diabetes vs placebo. This is reported as a study observation in the SURMOUNT-1 extension data set.
