A small-molecule GLP-1 you take like any other pill.
Orforglipron is a small-molecule (non-peptide) GLP-1 receptor agonist. The small-molecule structure survives stomach acid and digestive enzymes without an absorption enhancer, which is why no fasting window is required. Mechanism: activates GLP-1 receptors to reduce appetite, slow gastric emptying, and lower caloric intake.

GLP-1 receptor activation
Once absorbed, it activates GLP-1 receptors to help regulate appetite and food intake.

Six-step titration
Start at 0.8 mg, step up at least every 30 days through 2.5, 5.5, 9, 14.5, and 17.2 mg. Swallow whole, any time of day.
Side effects

How is Foundayo™ different from Wegovy® tablets?
Both are once-daily oral GLP-1s, but they are different molecules with different administration profiles. Wegovy® tablets contain a peptide (oral semaglutide) that requires an absorption enhancer and a strict empty-stomach routine — take with no more than 4 oz of water, wait 30 minutes before eating or drinking. Foundayo™ contains a small-molecule compound (orforglipron) that survives stomach acid on its own. You can take Foundayo™ any time of day, with or without food, with or without water restrictions. Your Maximus clinician will help determine which is the right fit for your goals and lifestyle.
What's the difference between intent-to-treat and completers?
Intent-to-treat (ITT) counts every participant who started the trial, including those who stopped early. Completers-only counts just the participants who finished the full 72 weeks. Foundayo™'s ATTAIN-1 trial reports 11.1% body weight loss (ITT) and 12.4% (completers) at the highest dose. Both are real numbers; the ITT figure is the more conservative one and is generally what regulators prefer.
I take oral contraceptives. What do I need to know?
Foundayo™ may reduce the effectiveness of oral contraceptives. If you take oral birth control, switch to a non-oral form or add a barrier method for 30 days after starting Foundayo™ and for 30 days after each dose escalation. This interval is longer than the standard 4 weeks for some other GLP-1s, so please discuss with your Maximus clinician before starting.
Can I take Foundayo™ for type 2 diabetes?
Foundayo™ is FDA-approved for chronic weight management only. Type 2 diabetes is not currently on the Foundayo™ label. Orforglipron has been studied for T2D in a separate clinical program (ACHIEVE), but that indication is not currently approved.
How long until I see results?
The ATTAIN-1 trial is reported at 72 weeks. Weight loss continues across the dose titration. Maintenance dose is 17.2 mg daily, reached after stepping through six dose strengths over several months at least 30 days apart.
