Tesamorelin: Your questions answered

By Starling Krentz, MS and Gabriel Alizaidy, MD, MS

Q: What is Tesamorelin and how does it work?

Tesamorelin is a peptide that signals your pituitary gland to release more of your own growth hormone. It is a synthetic analog of growth hormone releasing hormone (GHRH), meaning it mirrors the signal your hypothalamus naturally sends. Your body does the rest. That means you are not replacing anything from the outside. You are just turning up the volume on a system that already exists.

Q: How is Tesamorelin different from Sermorelin?

Both are GHRH analogs that work through the same basic pathway, stimulating your pituitary to release growth hormone on its own. The difference is potency and duration. Tesamorelin produces a stronger, more sustained growth hormone pulse than Sermorelin, which tends to translate into more meaningful IGF-1 movement and more noticeable body composition changes, particularly around visceral fat. If Sermorelin is a steady nudge, Tesamorelin is a firmer push. Same direction, more force.

Q: What does Tesamorelin actually do for body composition?

The most well-documented effect is visceral adipose tissue (VAT) reduction. That is the deep abdominal fat that wraps around your organs and drives metabolic risk. Tesamorelin has a clinical track record here that most peptides do not. Beyond fat loss, users often report better recovery, improved muscle preservation, and the kind of body recomposition that shows up slowly over weeks and months, not overnight.

Q: What role does IGF-1 play in determining if Tesamorelin is useful?

IGF-1 is the most practical lab marker for tracking growth hormone activity. Tesamorelin tends to move it more reliably than lighter peptides. If you start low, expect a meaningful upward shift toward mid-to-high normal. If you are already high-normal, you will hit a physiological ceiling. That said, labs are only part of the picture. Sleep quality, recovery between sessions, energy, and how your midsection is responding all matter just as much as the number on the report.

Q: How long before I see results?

Sleep and recovery changes tend to come first, often within the first few weeks. Energy follows. Visible body composition changes, especially around the midsection, take longer. Think in terms of months, not weeks, for the full picture. This is compounding interest, not a lottery ticket.

Q: Why wait 1 to 2 hours after eating before injecting Tesamorelin?

When you eat, insulin rises to shuttle nutrients into your cells. Insulin also blunts growth hormone release. Since Tesamorelin works by stimulating your natural GH pulses, you want insulin levels low to get the full effect. Dosing before bed after a few hours without food, or first thing in the morning after an overnight fast, gives you the best response.

Q: Can I use Tesamorelin while cutting or in a calorie deficit?

Yes, and it actually pairs well with a cut. In a deficit, insulin runs lower, which supports growth hormone pulsatility. IGF-1 on labs might not move as dramatically because the liver produces less of it in a calorie-restricted state, but that does not mean the peptide is not working. Recovery, muscle preservation, and fat loss can all still improve. The body composition benefits are real even when the bloodwork looks modest.

Q: What if Tesamorelin disrupts my sleep instead of improving it?

A small number of people experience vivid dreams or lighter sleep with nighttime dosing. If that happens to you, try shifting to a morning dose. Most of the body composition benefits are not dependent on the timing being at night, so you do not have to sacrifice results to find a schedule that works for your sleep.

Q: The injections sting a little. What can I do?

Let the syringe reach room temperature before injecting. Inject slowly. Rotate your sites. A small adjustment in angle or depth can make a real difference. If irritation is persistent or getting worse, flag it with your provider.
