Regulatory status
Enclomiphene is not listed as a controlled substance under the federal Controlled Substances Act07. It is a prescription medication. No FDA-approved enclomiphene finished drug product is available in the United States, so it is dispensed as a compounded preparation on a physician prescription.
Testosterone products, including injectable testosterone, are federally scheduled controlled substances and carry additional prescribing and dispensing requirements. Those requirements do not apply to enclomiphene, and enclomiphene's exemption from them does not lessen the need for a prescription or for physician supervision.

Mechanism
What enclomiphene is, and how it differs from TRT
Enclomiphene is a SERM. It blocks estrogen receptors at the hypothalamus. Your pituitary then releases more Luteinizing Hormone (LH) and Follicle Stimulating Hormone (FSH). Those hormones tell your testes to make more testosterone. LH and FSH keep working, so fertility markers typically hold.
Testosterone Replacement Therapy (TRT) runs the other direction. It supplies testosterone from outside. That can quiet the same signals. Enclomiphene starts the signal inside your body.

Study design and what the figures mean

Visual disturbances are rare and warrant discontinuation and provider evaluation. Elevated liver enzymes are rare. Thromboembolism risk is a rare theoretical class effect shared with clomiphene.

Reported effects across the class include acne or oily skin, libido changes, mood changes, fluid retention, elevated blood pressure, elevated hematocrit and hemoglobin, elevated PSA, testicular shrinkage, suppressed fertility, gynecomastia, and worsening of sleep apnea.
Serious risks include polycythemia or elevated hematocrit (CBC monitoring required), deep vein thrombosis and pulmonary embolism (rare), cardiovascular events in high-risk individuals, significant suppression of sperm production, and hypersensitivity reactions.

Topical secondary exposure. Testosterone Cream can transfer to another person through skin contact. Wash your hands after applying it and cover the application site with clothing. The site can be washed after 2 hours. Keep children and pets away from the site.
Injection-specific. Injection site pain, swelling, or bruising. Hypersensitivity to the oil carrier is rare. Injectable testosterone is a controlled substance.

Clomiphene, enclomiphene, and zuclomiphene appear in none of the five federal schedules. Enclomiphene is a triphenylethylene selective estrogen receptor modulator rather than a testosterone analogue, so it does not meet the anabolic-steroid definition that 21 C.F.R. § 1308.13(f) reaches. DEA publishes its schedules with an express note that the lists are not comprehensive, which is why this page states only that enclomiphene is not a scheduled controlled substance.

Enclomiphene is dispensed as an oral capsule, which is a nonsterile preparation, so <795> is the governing chapter. USP <797> covers sterile preparations such as injectables and does not apply to an oral capsule. The current revision of <795> became official November 1, 2023 and formalized beyond-use dating by dosage form.
