Blood Flow: Your Questions Answered

By Starling Krentz, MS and Gabriel Alizaidy, MD, MS

This guide breaks down everything clients have been asking about how these compounds actually work, their benefits, and what to expect.

Q: What are PDE5 inhibitors, and what's in the Blood Flow Protocol?

A: PDE5 inhibitors are compounds that increase nitric oxide and blood flow throughout the body. The Maximus Blood Flow Protocol offers four options: tadalafil, vardenafil, sildenafil, and a tadalafil + vardenafil combination. Each enhances circulation, performance, and recovery in different ways.

Q: Why do you call it the "Blood Flow Protocol"?

A: While PDE5 inhibitors are commonly known for treating erectile dysfunction, that's just one application of their core mechanism: improving blood flow and vascular function throughout the entire body.

We call it the “Blood Flow Protocol” because that's exactly what these compounds do at the physiological level. They enhance circulation to muscles, organs, and tissues systemwide. PDE5 is expressed not just in penile vasculature, but throughout the body including skeletal muscle, heart, lungs, and the vascular endothelium

Q: What's the difference between the three medications?

A: Each PDE5 inhibitor has distinct characteristics:

Tadalafil is the marathon runner. It provides around-the-clock vascular support for 24-36 hours, making it ideal for daily use and long-term cardiovascular benefits.

Vardenafil is the sprinter. The fastest-acting of the bunch at 10-15 minutes, highly selective for PDE5, and powerful at lower doses with fewer side effects.

Sildenafil (the same active ingredient as Viagra) sits in the middle. It works within 30 minutes but isn't as specific to PDE5. This lack of selectivity means more spillover effects and more common side effects like headaches, flushing, and nasal congestion compared to vardenafil.

Q: Why combine tadalafil and vardenafil?

A: The combination gives you both immediate and sustained effects. Tadalafil provides long-lasting vascular support while vardenafil delivers a quick, potent boost. Clinical data shows dual use can outperform single compounds for blood flow and recovery.

You get short-term enhancement and long-term cardiovascular support in one formula. The combo supports endurance, mental clarity, and sexual function without the crash or overstimulation of traditional pre-workouts.

Q: Why not just use sildenafil (the same active ingredient as Viagra) since everyone knows that one?

A: Vardenafil is nine times more potent at inhibiting PDE5 than sildenafil, meaning you get better results at much lower doses (2.5-5mg vs 100mg). This translates to fewer side effects like headaches and flushing. Sildenafil also has more spillover effects beyond PDE5, making side effects more common. Vardenafil is more selective and faster-acting.

Q: How long does it take to work?

A: Vardenafil typically kicks in within 10-15 minutes when taken sublingually, making it perfect to take on the drive to the gym. Sildenafil works in about 30 minutes. Tadalafil builds up over time, so daily use leads to stronger vascular and recovery benefits.

Some feel the difference on day one. Others notice the full benefits after a week of consistent use.

Q: How should I time the dose around training?

A: For vardenafil, take it 10-15 minutes before your workout. You can literally take it on the drive there. For the tadalafil + vardenafil combo, the timing is similar. Because tadalafil remains active for over 24 hours, consistent timing isn't essential. You can take it in the morning or evening depending on your schedule.

Q: What results can I expect?

A: Expect bigger pumps, better recovery, and sharper focus both in and out of the gym. Research shows PDE5 inhibitors can increase muscle protein synthesis, enhance oxygen delivery, and improve nutrient uptake.

Many users report faster recovery, better endurance, and easier strength progression. Studies have even shown an increase in reps during resistance training due to improved blood flow and oxygen delivery. There's also a cognitive benefit from increased cerebral blood flow, improving focus and processing speed. And yes, performance improves in every sense of the word.

Q: What's the benefit of the "pump" during workouts?

A: The pump isn't just cosmetic. Increased blood flow drives muscle growth by improving nutrient delivery, oxygenation, and cell signaling. Research shows that when cellular swelling and vascular delivery are optimized, muscle repair and growth signals are amplified.

This translates to better performance in the moment and faster recovery between sessions. Even NFL players know this. Chad Johnson (Ochocinco) used to talk about taking blood flow medication before games to get an edge on the field. If it's good enough for prime-time football, it's definitely good enough for leg day.

Q: Can it help with mental focus or cognitive function?

A: Yes. PDE5 inhibitors increase blood flow to the brain, improving oxygen and nutrient delivery. Studies show daily tadalafil improves attention, memory, and processing speed—not through stimulation, but through better cerebral perfusion. Users report sharper focus and sustained mental stamina throughout the day.

Q: What about prostate health benefits?

A: Research suggests that daily PDE5 use may reduce symptoms of benign prostatic hyperplasia (BPH), such as frequent urination or weak flow. While not prescribed for that purpose within the Maximus program, smooth muscle relaxation in the prostate and bladder can offer an added benefit.

Q: Can women use PDE5 inhibitors?

A: Yes. All three PDE5 inhibitors (tadalafil, vardenafil, and sildenafil) are safe for women to use. Female anatomy also contains PDE5 enzymes in vascular tissue, including in the clitoris and vaginal walls.

By increasing blood flow to these areas, PDE5 inhibitors can enhance sensitivity, arousal, and overall sexual function. Women also experience the same cardiovascular, cognitive, and workout performance benefits as men, including better pumps, improved endurance, and enhanced recovery.

Q: Are people taking this for longevity?

A: The "longevity" and "anti-aging" space is mostly marketing hype. That said, studies show PDE5 inhibitors improve endothelial function, reduce arterial stiffness, and enhance vascular health.

Better blood flow supports recovery in muscles, connective tissue, and organs. Some use low-dose daily tadalafil (2.5-5mg) as a cardiovascular optimization tool. Does it extend lifespan? Unknown. Does it improve cardiovascular markers? Yes.

Q: Is it safe to use long-term?

A: Yes. PDE5 inhibitors have been studied for decades and are considered safe for daily use at prescribed doses. They can improve cardiovascular health markers, endothelial function, and blood vessel elasticity.

Men with mild or managed conditions like sleep apnea can still safely use the protocol. It may even support vascular health and oxygenation during rest. Always start at the lower dose and consult your physician if you take blood pressure medication.

Q: Should I take breaks or cycle it?

A: If you want a break from enhanced pumps, sure, take a break. Otherwise, no need. PDE5 inhibitors don't build tolerance. Consistent daily use improves vascular tone and recovery over time.

Q: How does it affect blood pressure or heart rate?

A: PDE5 inhibitors slightly lower blood pressure through vasodilation, which benefits cardiovascular function. The effect is mild and well-tolerated. There's no significant increase in heart rate.

If you're on blood pressure medication, mention this to your doctor before starting. Always consult your doctor to ensure it's appropriate for your specific situation.

Q: Can I take it with caffeine or pre-workout?

A: Yes. Caffeine is a central vasoconstrictor in the brain but acts as a peripheral vasodilator during exercise. The rise in epinephrine plus local metabolic vasodilators during training overwhelm any vasoconstriction from caffeine.

Caffeine tends to enhance or preserve muscle vasodilation rather than restrict it. PDE5 inhibition is a much stronger dilator overall. They don't cancel each other out. Most users find the Blood Flow Protocol powerful enough on its own and no longer rely on typical pre-workouts.

Q: What's the bottom line?

A: The Blood Flow Protocol supports full-body performance in the gym, at work, and in the bedroom. By improving circulation, oxygen delivery, and vascular function, it enhances recovery, focus, and stamina in every domain.

Low side effects. No tolerance. Long-term benefits for cardiovascular and sexual health. Whether you're using it for acute performance, daily longevity support, or both, it's one of the most effective, science-backed tools available.
